SUMMARY:
This matter was filed on May 10, 2005, and a prehearing conference was held on May 16. The attorneys for the parties submitted statements of issues, which fully set forth the issues that were to be heard in this hearing. [Exhibits H.O.-2, H.O.-3] The hearing date of June 16 was cancelled, due to an unforeseen medical procedure involving one of the parties. A request for an extension of the mailing date of the decision was granted to allow for the scheduling of additional hearing dates.
